Health Care Flexible Spending Account...
Allied makes available a Section 125 Health Care Flexible Spending Account that helps pay the employee’s out-of-pocket medical costs while increasing spendable income. Under this plan, a specified amount of pre-taxed money is deducted from the employee’s paycheck each pay period. These dollars are set aside in a reimbursement account from which the employee can draw in order to pay for qualified health care expenses. For 2008, the maximum deduction amounts are $3000 for health care expenses and $5000 for dependent care expenses. (Employees: Please fax your completed enrollment form to us at 704-544-9005.)
Allied provides a 401K Retirement Plan through the Fidelity Group. The plan allows you to contribute a percentage of your gross wages through payroll deduction to an investment program, which is designed for retirement purposes. Your contributions are not subjected to income taxes until they are withdrawn. Early withdrawal is possible under certain conditions, but there may be a penalty. The plan is designed and intended for retirement and does not lend itself readily to early withdrawals or for loans. You are eligible for the 401K Retirement Plan on the first day of the month following six months of continuous employment.Enrollment is done online through the Fidelity website or by phone with a representative.Please be sure to let Allied know directly that you have enrolled plus your weekly percentage that you choose. Also, the only form that Allied needs is the completed Designation of Beneficiary Form, which is included in the enrollment package.
